Flavio Boltro grew up in a house where the trumpet was already in the room. His father played it and loved jazz, his mother taught school, and the son started lessons at nine. At thirteen he entered Turin's "G. Verdi" conservatory, where he spent seven years studying under Carlo Arfinengo, the principal trumpet of the city's symphony orchestra.

The first record to carry his name is not a jazz album. In 1979, at eighteen, he played trumpet on Quinto Stato, the third studio album by Arti e Mestieri, the Turin progressive rock and jazz-rock group that drummer Furio Chirico had founded in 1974. He returned to the band for Children's Blues in 1985. Between 1982 and 1985 he also held chairs in Turin's symphonic orchestra and the RAI symphonic orchestra. Conservatory discipline, a prog-rock rhythm section and an orchestral pit were all part of his training before he turned twenty-five, and the breadth of what he has recorded since is easier to read once that is known.

Jazz took over in the middle of the 1980s. He played in quintets with the American tenor saxophonist Steve Grossman alongside Cedar Walton and Billy Higgins, and worked with Clifford Jordan and Jimmy Cobb. From 1984 to 1986 he was in Lingomania, the group Maurizio Giammarco led with Roberto Gatto on drums, Furio Di Castri on bass and Umberto Fiorentino on guitar. Somewhere in the same stretch he recorded the trumpet solo heard in the opening theme of Casa Vianello, the Sandra Mondaini and Raimondo Vianello sitcom that ran from 1988 to 2007 on Canale 5 and then Rete 4, which remains the most widely heard thing he has played and the one credit that carries no name.

His first album as a leader, Immagini, came out on Red Record in 1987, a trio with Di Castri and drummer Manhu Roche that Boltro produced himself. The trio added a fourth voice, Joe Lovano, and in December 1988 the four went into Studio Emme in Florence. Part of that material appeared under Di Castri's name on Unknown Voyage, which also brought in Paul Motian and Franco D'Andrea. The rest stayed unreleased until October 2025, when Musica Jazz issued nine previously unheard tracks from those sessions as Boss Town, attached to that month's magazine. Thirty-seven years separate the session from its release.

France then took two decades of his working life. In 1994 Laurent Cugny brought Boltro into the Orchestre National de Jazz, the French state jazz orchestra, alongside the saxophonist Stefano Di Battista; three ONJ albums followed on Verve between 1996 and 1997. Michel Petrucciani took him into his sextet from 1996 to 2000, the band that recorded Both Worlds in 1997. In 1997 Boltro and Di Battista formed their own quintet with Eric Legnini on piano, Rosario Bonaccorso on bass and Benjamin Henocq on drums, and made Volare, which Label Bleu reissued on vinyl in 2016. Blue Note's French arm signed Boltro for two albums of his own, Road Runner in 1999 and 40° in 2003. In the same year the Académie du Jazz named (((Air))), his trio record with pianist Giovanni Mirabassi and the American trombonist Glenn Ferris, its Album of the Year.

He kept his Italian working relationships running in parallel. From 2005 to 2009 he played in Rosario Giuliani's quintet, including Anything Else on Dreyfus in 2006. In Rome on 3 and 4 October 2005 he recorded Trumpet Legacy with Fabrizio Bosso, a two-trumpet front line over Luca Mannutza, Luca Bulgarelli and Lorenzo Tucci. He appears on Milestones: Un Incontro in Jazz (Blue Note, 2007) with the singer Gino Paoli, Enrico Rava, Danilo Rea, Bonaccorso, Gatto and Renato Sellani, and on three albums by Nicola Conte. He recorded four albums with Giovanni Tommaso's quintet between 1988 and 1998, and carries pop credits with Francesco De Gregori, Francesco Baccini and Laurent Voulzy.

Opera is the strand he keeps returning to. On 9 December 2010, at Schloss Elmau in Bavaria, Boltro and Danilo Rea recorded a duo album for ACT made up entirely of Italian opera: twelve pieces drawn from Monteverdi, Rossini, Bellini, Puccini and Cilea, among them "E lucevan le stelle," "O mio babbino caro" and "Casta Diva." Released on 24 June 2011 as Opera, it puts the trumpet where the voice would be and leaves the aria to survive on phrasing and breath. He came back to the idea in 2024 with Veste Rosa on Casa Editions, a quintet programme built entirely on Giuseppe Verdi with Gavino Murgia on saxophone, Giuliano D'Amico singing, Fabio Giachino on piano and Davide Liberti on bass.

The recent work runs in three directions at once. Things To Say (CAM Jazz, 21 April 2022) is a duo with Giachino, cut at Riverside Studio in Turin in December 2020 with liner notes by the critic Brian Morton. Letting Go (Igloo Records, 8 June 2023), with the Luxembourg guitarist Greg Lamy, Gautier Laurent and Jean-Marc Robin, was recorded entirely in first takes according to the label. Between them came Brigà, released by Casa Editions in 2023 and grown out of a 2018 residency at CNCM Voce in Pigna, Corsica, where Boltro's BBB Trio with Mauro Battisti and Mattia Barbieri met the singer and clarinetist Jérôme Casalonga and the electronic composer Malik Berki. It is a trumpet trio with no harmony instrument, a Corsican voice and electronics, and it belongs to the same musician who recorded Verdi the following year.

Boltro plays with a lyrical, singing tone and builds solos out of long melodic lines rather than dense harmonic running, an approach that connects free improvisation to modal jazz, post-bop, folk material and rock and pop. The conservatory training under Carlo Arfinengo left him with an orchestral player's control of sound, and the years inside Turin's symphonic orchestra and the RAI symphonic orchestra reinforced it. What separates him from a straight-ahead hard bop trumpeter is where he takes that tone: into a progressive rock band at eighteen, into a state jazz orchestra in Paris, into Corsican polyphony with electronics, and twice into Italian opera, where he takes the vocal line himself.

The two-trumpet meeting with Fabrizio Bosso on Trumpet Legacy is the clearest place to hear what his sound is and is not, since the two players are audibly different inside the same front line. His French period sits under the influence of the bands he was in rather than of any single player: Michel Petrucciani's sextet, Laurent Cugny's Orchestre National de Jazz, and the quintet he co-led with Stefano Di Battista, all of which pushed him toward writing and arranging as well as playing.
